Frequently Asked Question

1. From the Home screen within the CHADIS system, enter the patient’s name in the search window and hit the "Enter" key on your keyboard.
2. Select the patient you are looking for by clicking on the magnifying glass to the left of the patient’s name
3. The details window for the patient you have selected will then appear. Items on the details page include:

• Vital Information - Gender, Birth Date, Telephone Number.
• Latest Reports - List of the most recent reports that were created for the patient in question.
• Respondents - Individuals connected to account that have taken or have the ability to take questionnaires about the patient in question.
• Questionnaires - List of questionnaires that need to be taken and where questionnaires can be added for the patient in question.
• Reports – Comprehensive list of reports that have been created for the patient in question.
