How Patients and Respondents Are Registered in CHADIS (Epic Integrated Clients)

After going live, your patients in Epic will automatically sync to CHADIS as appointments are processed.
1. To view the patient file in CHADIS, log into CHADIS and click on ‘All Patients’.
2. Search for the patient name or DOB.
3. Click on the patient name to open up the CHADIS patient file. The patient information (synced from Epic) is displayed at the top. ‘External Patient Identifiers’ shows the Epic MRN of the patient. The ‘Respondents’ section displays all respondents for the patient. The number next to the respondent shows how many questionnaires are assigned to that respondent..
4. The ‘Questionnaires’ section includes recently assigned and submitted questionnaires.
