Q: How do I schedule a patient appointment now that we’re integrated?

A: Patient appointments are scheduled the same as usual.


Q: How do I order CHADIS questionnaires?

A: CHADIS will view the schedule and automatically send questionnaires to patients before their appointments via email, based on visit type and age of the patient. The CHADIS team will help you map your appointment types and questionnaires when the integration is set up.


Q: When will patients receive their questionnaires?

A: CHADIS will send questionnaires to patients via email approximately 1-2 weeks in advance of appointments.


Q: How often does CHADIS check the schedule?

A: CHADIS will check the schedule multiple times throughout the day to catch any appointments that may have changed and will check more frequently during the current business day.


Q: Where are the results of the questionnaires returned?

A: As soon as questionnaires have been completed, CHADIS automatically scores the questionnaires and results are ready to be viewed by office team via EPIC.


Q: What if parents/patients don’t complete their questionnaires before a visit?

A: Questionnaires can be completed in the waiting room via a PC, tablet, or smartphone. Staff can also assist patients by using the login switch option in CHADIS.


Q: How quickly will results be returned if the questionnaire is completed in the waiting room?

A: CHADIS processes questionnaires very quickly and will return the results to Epic within minutes of the questionnaire being submitted so the provider can review the results during the appointment.


Q: How are questionnaires assigned to teachers and how do they complete them?

A: Staff or parents can invite a teacher to complete a questionnaire by logging in to CHADIS, going to the patient’s account, selecting the “Invite” hyperlink, and then filling in the teacher’s information, including email address. The teacher will be notified via email they have a questionnaire to complete, can quickly create a CHADIS account, and complete the questionnaires assigned to them.


Q: How are questionnaires assigned to teens and how do they complete them?

A: Staff or parents can invite a teen to complete a questionnaire by logging in to CHADIS, going to the patient’s account, and adding the teen as a respondent. Once these steps have been completed the questionnaires will be sent to the teen via email. The teen will need to click on the link in the email, quickly create a CHADIS account, and complete the questionnaires assigned to them.


Q: Once we’re integrated, can parents still log in to CHADIS directly to complete questionnaires as usual?

A: Yes, parents with existing CHADIS accounts can complete questionnaires by logging in at www.chadis.com as usual.


Q: How does billing work with the integration?

A: Questionnaires are billed for the same as other procedures by manually adding the order to the encounter/bill as usual.


Q: Which questionnaires will be available via the integration?

A: Please review the attached list for details. You may also check specific questionnaires in CHADIS to confirm EHR support.


Q: Once we’re integrated, how do we enroll patients in CHADIS?

A: Once CHADIS is integrated with Epic, questionnaires will be automatically sent to patients via email. If patients are new to CHADIS they, or their caregiver, will be prompted to register their CHADIS account before taking the questionnaires.


Q: Is CHADIS connected to the portal?

A: No, not currently.


Q: Can questionnaire reminders be sent via text message to patients?

A: Yes, questionnaire reminders can be texted to patients.


Q: What if an appointment was canceled and rescheduled, will the results from the completed questionnaire be lost?

A: NO, the results will be moved to the rescheduled appointment/encounter.


Q: What if an appointment was canceled but no appointment was rescheduled, what will happen to those questionnaires results?

A: The results will still be stored in the Review Flowsheets section in EPIC.
