Respondents with Multiple Patients (CHADIS Direct Clients)

If a parent or teacher has multiple patients in your practice, they should only have one CHADIS account. Therefore, if you add them as a respondent for another patient in your practice, CHADIS will recognize the email address and/or mobile number and offer you options from which to select an existing account. 


1. Open up the patient chart in CHADIS, scroll down to the respondents section and click on the plus sign to add the existing respondent.

2. Enter the respondent’s name, relationship, and email address or phone number used to set up the account. Click ‘Next.’

3. CHADIS will prompt you to connect the existing respondent to the new patient. Click on the respondent to connect.

4. The respondent has now been linked to the new patient. You can click on the patient name to return to the CHADIS patient chart, auto-assign questionnaires, or use one of the waiting room options for this respondent.

5. You can also click on ‘View this respondent’s patients’ to confirm that both patients are connected to the respondent.
