Your CHADIS office administrators have the ability to create a questionnaire favorites list to easily access and manually assign your most frequently used questionnaires.
Log in to CHADIS and click on ‘Home’ under ‘Office Administration’ on the lefthand side navigation bar.
3. Click on ‘Add Favorite’ to add questionnaires to your favorites list.
4. Search for questionnaires by name or ID# and check the box next to the questionnaire to add it to Favorites. Multiple questionnaires can be checked and added at once. Click ‘save’ when you are finished.
5. Click ‘Alphabetize Favorites’ to quickly alphabetize your list, or click and drag to move questionnaires to the preferred order. Click the red trashcan to delete any unwanted questionnaires.
6. Your questionnaire favorites list will appear at the top of the page when manually assigning questionnaires to a patient.
