The Conners 4 questionnaires provide additional insights from a single administration, with results displayed numerically, graphically, and in an interpretive summary known as the Single-Rater Report. This report is distinct from the CHADIS results report, which includes only raw scores and item-level responses. The external Single-Rater Report can be accessed directly through the CHADIS report.
1. Log into CHADIS and click on ‘All Patients.’
2. Search for the patient using name or DOB.
3. Click on the patient to open up the CHADIS patient file. Click ‘Create New Report’ at the top of the page.
4. You will see a list with the recently submitted questionnaires. If no changes are needed, you can immediately click ‘Create Report.’ If you would like to edit the details, you can click the pencil icon in the top right corner and view all submitted questionnaires for this patient.
5. The CHADIS detailed results report will be generated and you can scroll down to see the scores. Make sure to click ‘include’ under the ‘Responses’ section to include the questions/answers as well.
6. To view the external Conners 4 Single-Rater Report, click on the Conners 4 logo next to the name of the questionnaire.
7. The Conners 4 Single-Rater Report will open in a new window to view, download, or print the report.
8. You can access this report at any time in the CHADIS patient file under the ‘Latest Reports’ section.
