Single Rater Report: The Conners 4 questionnaires provide additional insights from a single administration, with results displayed numerically, graphically, and in an interpretive summary known as the Single-Rater Report. A youth’s scores are compared to those in one or more Normative Samples, and optionally, to an ADHD Reference Sample. Also includes a Within-Profile Comparison of the youth’s results to their own average score. Elevations at both the scale and item level are included. Key results are provided in a one-page overview.
Multi-Rater Report: Combines the results of up to five raters to provide an overview of the youth’s behavior from a multi-rater perspective and highlights potentially important inter-rater differences in scores. It is available for the full-length Conners 4, Conners 4–Short, and the Conners 4–ADHD Index.
These reports are distinct from the CHADIS results report, which includes only raw scores and item-level responses. The external reports can be accessed directly through the CHADIS report.
1. Log into CHADIS and click on ‘All Patients.’
2. Search for the patient using name or DOB.
3. Click on the patient to open up the CHADIS patient file. Click ‘Create New Report’ at the top of the page.
4. You will see a list with the recently submitted questionnaires. If no changes are needed, you can immediately click ‘Create Report.’ If you would like to edit the details, you can click the pencil icon in the top right corner and view all submitted questionnaires for this patient.
5. If there are multiple Conners 4 questionnaires in the report that are eligible for a multi-rater report, you will see an additional page to confirm you'd like to create this report.
6. The CHADIS detailed results report will be generated and you can scroll down to see the scores. Make sure to click ‘include’ under the ‘Responses’ section to include the questions/answers as well.
7. To view the external Conners 4 Multi-Rater Report, click on the Conners 4 logo next to the Conners 4 Multi-Rater report under 'Conners 4 Reports'.
8. To view the external Conners 4 Single-Rater Report, click on the Conners 4 logo next to the name of the questionnaire.
7. The Conners 4 reports will open in a new window to view, download, or print the report.
8. You can access this report at any time in the CHADIS patient file under the ‘Latest Reports’ section.
