Frequently Asked Question

To preview a questionnaire before you choose to use it:

1. From the Home screen within the CHADIS system, click on the green "View Questionnaires and Resources" Button.
2. The list of available questionnaires will appear in Categorical order, but two additional views are available to preview the questionnaires as well: alphabetical listing and billing code listing.
3. Scroll down to select the questionnaire that you want to view. (Example: Ages & Stages Questionnaires®, Third Edition (ASQ-3™): 10 months Questionnaire) .
4. Read the overview information about the questionnaire and choose “Preview this Questionnaire” from the bottom of the screen to view the questionnaire.
5. To return to the home page, click "home" in the upper left hand corner of this page.
