*Reset a respondent's password

1. From the Home screen within the CHADIS system, click in the Search box and search for the Patient (not the respondent)
2. Click on the magnifying glass to the left of the patient’s name.
3. Within the details window, scroll down to Respondents and click on the Respondent’s name.
4. Click on “Reset this User’s Password”.
5. Enter a new password for this User. We recommend you use the same password every time – i.e., “Practice1”.
6. Inform Parent of New Password.
