Message Sent to Teachers Upon Invitation to CHADIS

When a teacher is invited to use CHADIS, CHADIS sends them an email. Please see an example of the email below to get an idea of what they will see. Please note that the patient's name is NOT provided in the email to ensure HIPAA compliancy. The email will state the name of your practice. The verbiage of the email body may be slightly different depending on whether the invitation came from your staff or from the patient (or parent).

Practice Name requests your input using CHADIS questionnaires for one of your students' upcoming healthcare visit. CHADIS uses validated questionnaires (like the Vanderbilt) to help educators and families communicate with doctors. All answers are confidentially delivered to the patient's doctor. CHADIS is FERPA compliant, so you will see the student’s name once you use the link to log in.

We received your information from the student's parent or caregiver who is requesting that we send this invitation directly to you. The parent/caregiver has given consent for you to complete this information via our secure website.
