Frequently Asked Question

CHADIS has a Programming Team which can customize a questionnaire/form to meet your needs. Costs vary depending on the complexity and detail of the requested form (i.e. scoring requirements, diagnoses, skip logic, etc.).

If after reviewing the pricing by tiers document you would like to request a questionnaire/form be customized in CHADIS, please complete and return the attached request form along with the specific questionnaire/form you want customized and any accompanying documents (i.e. translated version) to your Account Manager or designee. Be sure to provide as much detail as possible as is related to the purpose and desired outcomes.

The Custom Programming Request Form must be completed for each questionnaire/form requested.

Once you complete the form and send the accompanying document(s), your Account Manager will submit your request after which the Programming Team will provide a time/cost estimate to produce it in CHADIS.

Once you approve, there's an agreement to sign and the invoice will be generated. Once payment has been received, the Programming Team will begin developing the questionnaire desired.
