Nationally, most families referred for developmental services never receive them and practices are usually unaware of the status of the referral. As you know, the earlier intervention services are begun for delays or autism symptoms the better the outcome.

We created a free referral and tracking system in CHADIS for Montgomery County families and practices.

If your practice is in Montgomery County, Maryland, and is having trouble with this feature, please file a ticket.

Using CHADIS “Refer button” and tracking system:

  1. You will get a weekly email report of failed ASQ or MCHAT screens with CHADIS IDs. Enter the ID in the patient search in CHADIS to review results and refer as appropriate. You may also see fails in your CHADIS detail view of your CHADIS report when with your patients.
  2. Click the link that appears in the CHADIS table of results when there is a positive ASQ or MCHAT. A simple referral form will appear pre-filled with the fax number for Montgomery County Infants & Toddlers, family contact information, and area of the screen that was failed.

  3. Confirm they are residents of Montgomery County.
  4. Confirm parent's verbal permission for the referral, for Infants and Toddlers to contact them, and to share information. Or click to use the 'written consent' option parent signs by typing their name.
  5. Click 'Create' at the bottom of the page. The referral will automatically be e-faxed to Montgomery County Infants and Toddlers via CHADIS. If you wish to refer elsewhere, change the fax number.
  6. Tracking function is activated by when you create the referral:
    1. Alerts families referred one month later to complete a questionnaire about the status of evaluation and intervention. Results appear in child’s CHADIS chart.
    2. Infants and Toddlers agency is asked to send your office feedback about the services they delivered as part of the e-faxed referral.
    3. A spreadsheet with the status of patients referred using the button will be sent (id only, no PHI) to your office at intervals.
