CHADIS offers graphic display of questionnaire scores for a few different tools, including the Vanderbilt, Pediatric Asthma Control & Communication Instrument (PACCI), and some versions of the PHQ-9.

1. Within a report in the Questionnaires table, this icon will appear next to questionnaires that offer graphics:

2. Click that icon to open the graphic in a new window.

3. Alternatively, you can scroll down and view the graphic in the Charts section.
