To prepare for CHADIS implementation, your providers will need to choose the questionnaires you will be using for each visit type in your office. Providers can preview all available questionnaires in CHADIS to assist in making their decisions. Follow the steps below:
Log into CHADIS and click ‘View Questionnaires’ under ‘Resources’ on the home page.
2. You can search for a questionnaire by typing in the search bar, or choosing one or more filters in the left hand column. The filters include Age Group, Respondent, Use, Topic, Billing Codes, Languages, Other.
3. Click on the questionnaire to view more information on scoring, recommended ages, and preview the questions and response options.
4. Click ‘Preview this Questionnaire at the bottom of the page.
