Enter Prematurity Value for Patient (CHADIS Direct Clients)

CHADIS will age-adjust for developmental age vs. chronological age, up to age 2, per AAP recommendations for all developmental questionnaires. When completing registration, respondents will be asked if their patient is premature and enter the number of weeks premature. This data can also be entered by staff by editing the patient file. 

When using CHADIS auto-assignments to assign questionnaires (either staff auto-assign or respondent self auto-assign), CHADIS will adjust to gestational age for all developmental questionnaires. For example, if a child is 2 months old but 4 weeks premature, CHADIS will consider them to be 1 month old when assigning developmental questionnaires based on auto-assignments.

Below are instructions for staff to manually enter the prematurity value. 


1. Log into CHADIS and click on ‘All Patients.’

2. Search for the patient using name or DOB.

3. Click on the patient to open up the CHADIS patient file. Click on the 3 dots in the upper right corner of the patient section, and choose ‘Edit.’

4. Scroll down to ‘Premature Birth’ to choose prematurity status. If ‘Yes’ is chosen, enter the number of weeks premature and click ‘Save.'

5. The prematurity value will appear in the patient information section.
