CHADIS automatically assigns questionnaires based on a child’s age and the type of visit (i.e., Well Child Visit).

This default set of questionnaires and visit types is called Automatic Assignments.

The Automatic Assignments can be tailored specifically to your practice’s preferences but, only an office administrator can make changes. The questionnaires in your automatic assignments have been selected and approved by your office. They are unique to your office based on the selected visit types.

Please follow the steps in the attached handout, on the right, to view, edit, add, and remove auto assignments.
