Login Switch -Tablet (PCC Integrated Clients)

If the respondent does not complete their questionnaires before the appointment, CHADIS questionnaires can be completed in the office on a phone or tablet by logging into their PCC portal OR by using the CHADIS ‘Waiting Room Workflow’. The CHADIS Waiting Room Workflow allows the respondent to access CHADIS questionnaires quickly without typing in their username or password.


Login Switch allows you to switch the log in from the current staff user to the respondent user on the device you are using.

A common option is to log in as a staff on a tablet, login switch to the respondent and pass the tablet to the respondent.


1. Log into CHADIS and click on ‘Waiting Room’ on the home page.

2. Search for the patient using name, DOB, or PCC ID#, then click on the patient to open the waiting room options.

3. Click on the left icon, ‘Login Switch,’ next to the respondent with incomplete questionnaires to log in the respondent on the device you are using. This option is useful if you are logged in as a staff member on an office tablet and would like to switch the login to a respondent.

4. CHADIS will prompt staff to pass the device to the respondent user so they can access CHADIS and complete their questionnaires. They do not need to type in their username or password.
