The age cut-off is the age at which you want teens to start taking their own questionnaires. Once determined, this age cut-off applies across the practice for all visit types and questionnaires. The age cut-off must be identified so that when entering questionnaires, the max age for primary caregiver does not exceed the age cut-off and the min age for self cannot start before the age cut-off. That is why you may need to duplicate the questionnaire entries, so that it will get assigned with the integration, which recognizes only one respondent (primary caregiver OR self).
Example: PHQ-9 taken from 8 years to 19 years, but age cut-off is 12.
