How Patients and Respondents Are Registered in CHADIS (Athena Integrated Clients)

There will be no CHADIS charts when you first go live.

Patients will be automatically registered in CHADIS (charts created) when CHADIS sees mapped appointments in Athena that are within the lookout period.

Once the chart is created, CHADIS will send a welcome and register notification to the respondent whose contact information comes from Athena (portal sign-up email, email address, mobile number), instructing them to REGISTER and take questionnaires.


It is CRITICAL that respondents be informed to use the link in the notification to REGISTER. They must be registered in order to take questionnaires prior to the visit. 

NOTE: All respondents must be set up before they can take questionnaires. If they did not register with CHADIS, staff will need to add them as a respondent in order for them to be able to take questionnaires in the office. They will then receive a registration link to set up their CHADIS account to take questionnaires prior to future visits.

Note: Educate parents to look for the Welcome email and register using the link provided. They must be registered in order to take questionnaires prior to the visit. Set the expectation from the start.
