CHADIS checks the Athena schedule throughout the day and if a mapped appointment type is booked WITHIN the next 2 weeks, CHADIS will assign the appropriate questionnaires based on the CHADIS visit type and patient’s age AT the time of assignment. 

If respondent contact information comes over from Athena, CHADIS will notify the respondent that there are questionnaires to take. The respondent will then click on the link in the notification to register/set up their account and/or log into CHADIS with their username and password and take the assigned questionnaires.

Upon the FIRST notification from CHADIS, respondents must be instructed to complete their CHADIS Registration. They must complete their registration in order to take questionnaires prior to the visit!

Once registered, they will be taken to the questionnaires which have been assigned.

1. First time users will agree to the Terms of Use.

2. They will be taken to their Home page and will see how many questionnaires there are to take. Click ‘Take Questionnaires.’

3. Click the first questionnaire to begin.

4. Click ‘Send it. I’m Done’ when done. The results are immediately scored and the results and responses are sent to the Athena chart. 

5. The next questionnaire will automatically appear for the respondent to take.
