Manually Assign Questionnaires (Athena Integrated Clients)

You can always manually assign an individual questionnaire to a patient at any time (separate from your auto-assignments). This is done in CHADIS; therefore, you can assign to any age as the age cutoff does not apply.


1. From the CHADIS patient chart, Click ‘Assign Questionnaires’ at the top and choose ‘Manual.’

2. Specify the doctor and the respondent from the drop-downs. If you leave it at ‘Any Respondent’, then any respondent has access and it will be completed by the first person to access it, but if you assign to a specific person, only that person has access. Checking ‘Send assignment notification’ will send an email/text to the respondent letting them know they have questionnaires to complete, providing a link for them to log in and complete them. If they are in front of you, you do not need to send notification.

3. Search for the desired questionnaire by title or ID#. Once you have selected the questionnaire you want to use, check the box next to it. You can select multiple questionnaires if you wish.

*NOTE: If you manually assign certain questionnaires often, you may consider adding them to your auto-assignments. Or, it would be a good idea to keep a list of questionnaires you plan to use often and indicate their unique ID#. Many questionnaires have similar names, so it will help to distinguish the right one when making a selection.

4. You can also click the magnifying glass to the right of the questionnaire to open up a pop up window that gives details on that questionnaire:

5. Once you have chosen the questionnaires you would like to manually assign, scroll back to the top and click ‘Save’.

6. Now those questionnaires have been assigned and will show up in the patient’s chart as ‘New,’ assigned, but not yet completed:

Questionnaires show in ascending order; the most recently assigned questionnaires will be at the bottom. This is important to note as the questionnaires at the bottom of the list are those associated with the most current encounter.
