Reasons Questionnaires Were Not Assigned (Athena Integrated Clients)

There are valid reasons why questionnaires were not assigned.

  1. The Athena appointment type is not mapped to a CHADIS visit type.

  2. The patient did not meet the age criteria set up in the auto-assignments. Age is calculated AT the time of notification from Athena to CHADIS.

  3. The prematurity field was populated with a value; therefore, CHADIS will age adjust for developmental vs. chronological age, up to age 2, per AAP guidelines.

  4. The respondent’s relationship to the patient is not entered correctly.

  5. The respondent’s relationship listed in Athena is not programmed in CHADIS (ie. caretaker, foster parent, etc.). Contact your account manager to have that unique relationship added.
