CHADIS will age-adjust for developmental age vs. chronological age, up to age 2, per AAP recommendations for all developmental questionnaires. However, in order for CHADIS to age-adjust, the number of weeks premature must be manually entered under the Vital Information section of the CHADIS chart. This information does NOT come from Athena.
1. In the CHADIS patient chart, click on the 3 dots in the upper right corner of the patient section, and choose ‘Edit.’
2. Scroll down to ‘Premature Birth’ to choose prematurity status. If ‘Yes’ is chosen, enter the number of weeks premature and click ‘Save.'
3. The prematurity value will appear in the patient information section.
4. CHADIS will read the prematurity value for all FUTURE visits. However, CHADIS checks Athena and assigns questionnaires only one time per visit. Since prematurity is unknown the very first time CHADIS assigns questionnaires, CHADIS assigns based on chronological age. Therefore, you may need to manually assign the developmental questionnaire (ie. ASQ) and remove the chronological questionnaire that was auto-assigned. Again, you only need to do this the first time. Once the prematurity field is populated, CHADIS will age-adjust for all future visits.
