The default prematurity adjustment is ONLY for developmental questionnaires; that is, if the prematurity value field is populated, the age will be adjusted only for developmental questionnaires (ie. ASQ, MCHAT, etc.).

**If you would like to turn off CHADIS prematurity adjustment for a questionnaire, contact your Account Manager to allow you to alter the default settings for that questionnaire. Once they have enabled the ability to change the default settings, follow the instructions below.

1. Log in to CHADIS and click ‘Automatic Assignments’ under ‘Office Administration’ on the left hand side.

2. Click on the questionnaire to edit the prematurity setting.

3. You can now select ‘Use Default’ (only developmentally related questionnaires are adjusted), ‘Always Adjust’ or ‘Never Adjust.’
