Frequently Asked Question
Although questionnaires are automatically assigned based on your CHADIS auto-assignments, staff can also manually assign additional individual questionnaires to patients at any time. This may be useful if the provider would like to screen for any other health concerns not in the typical rotation for the visit.
1. From the progress notes in eCW, click the CHADIS button to open up the CHADIS patient file. Click ‘Assign Questionnaires’ at the top of the patient file and choose ‘Manual.
2. Choose the respondent, the provider, and search for the questionnaire to select it. You can also click the magnifying glass to the right of the questionnaire title to view more details and a preview of the questionnaire first. Leave ‘Send assignment notification’ checked to send the respondent an email and/or text notifying the respondent that they have a questionnaire to complete.
3. Search for the desired questionnaire by title or ID#. Once you have selected the questionnaire you want to use, check the box next to it. You can select multiple questionnaires if you wish.
4. You can also click the magnifying glass to the right of the questionnaire to open up a pop up window that gives details on that questionnaire:
5. Once you have chosen the questionnaires you would like to manually assign, scroll back to the top and click ‘Save’.
6. After clicking ‘Save,’ the questionnaires selected will appear as assigned in the ‘Questionnaires’ section.
The results will go into eCW as an addressed telephone encounter and is available in the patient documents.
