Reasons Questionnaires Were Not Assigned (eCW Integrated Clients)

There are valid reasons why questionnaires were not assigned.

  1. The eCW visit type is not a mapped visit type

  2. The patient did not meet the age criteria which is set up in the auto-assignments, AT the time of notification from eCW to CHADIS.

  3. The prematurity field was populated with a value; therefore, CHADIS will age adjust for developmental vs. chronological age, up to age 2, per AAP guidelines.

  4. The respondent’s relationship to the patient is not entered correctly.
