Adolescents can use a portal account to access CHADIS questionnaires, but staff can also invite teens to create a CHADIS account.

Below are instructions for staff to invite teens to CHADIS directly. 

1. In the CHADIS patient chart, scroll down to the Respondents section and click on the plus sign to register a teen respondent.

2. Choose the relationship ‘Self/Patient’ and CHADIS will fill in the patient’s name. Enter the teen’s phone number or email address, and click ‘Next’ to send them a registration message inviting them to CHADIS.

3. A CHADIS registration message has now been sent to the teen. You can now assign questionnaires or use the waiting room options.

4. The teen will receive a registration message for CHADIS and complete their registration using the link.
