OP Template Assignments should be added for the questionnaires that are programmed to be assigned to every patient for certain visits. These questionnaires are added as standing orders to OP Well Visit or Encounter templates. *Use template assignments to make assigning via the waiting room easier. That way, the patient and questionnaire details have already been sent to CHADIS*

In OP:

1. Add the default CPT code in the Survey Administrator. For updated billing information, please see the CHADIS knowledgebase.

2. Add the CHADIS questionnaires as standing order in the OP templates
