After going live, your patients in OP will be gradually added to CHADIS via OP order. This means that CHADIS will not have a patient registered if the patient does not have an upcoming appointment. You may always access the patient record in CHADIS by pushing the CHADIS button in OP (skip to step 3) or you can also log directly into CHADIS.
1. Click on the CHADIS button from the patient record in OP to open up the CHADIS patient file (skip to step 3), or log into CHADIS directly and click on ‘All Patients’.
2. Search for the patient name or OP ID#. Click on the patient’s name to enter the CHADIS patient file.
3. The patient information (synced from OP) is displayed at the top. ‘External Patient Identifiers’ shows the OP ID# of the patient. The ‘Respondents’ section displays all respondents for the patient which includes any OP portal users (synced from OP) and any teacher respondents. The number next to the respondent shows how many questionnaires are assigned to that respondent.
4. The ‘Questionnaires’ section includes recently assigned and submitted questionnaires.
