Manually Task Surveys: OP
In most cases, the CHADIS surveys will be automatically assigned from standing order on the templates. Your clinical team may also assign individual questionnaires to a patient. This is a workflow exception that allows someone to select questionnaires from the Task section in OP. A visit is not required to manually assign a CHADIS questionnaire from OP. The surveys may be available to the respondent in the Patient Portal, and the results will populate in the patient’s chart in OP.
1. Log into Office Practicum with your username and password.
2. Search and select the appropriate patient’s chart
3. Manually add the survey(s)
Click on Task tab
Click Orders
Survey tab
Select the CHADIS survey
Change the department to patient
Click “Create Task/Order to do surveys
4. Parents/respondents with a portal account will receive a notification from the portal and will see the surveys listed in their portal account. You may also transition to the Waiting Room workflow if the parent/respondent cannot access the IC Patient Portal.
5. The results of the completed survey will populate in OP in the patient’s chart in the Survey tab.
6. Reminder: To push the surveys to the superbill, providers must change Status fields to complete.
