Frequently Asked Question

Results should appear in OP within 5 minutes after the respondent has submitted the questionnaire. However, occasionally, you may need to download the results directly from CHADIS.

1. Click the CHADIS button from the patient’s chart in OP to open up the CHADIS patient file. Click ‘Create New Report’ at the top of the page.

2. You will see a list with the recently submitted questionnaires. If no changes are needed, you can immediately click ‘Create Report.’ If you would like to edit the details, you can click the pencil icon in the top right corner.

3. The CHADIS detailed results report will be generated and you can scroll down to see the scores. Make sure to click ‘include’ under the ‘Responses’ section to include the questions/answers as well.

4. When the report is ready, use the ‘Print’ function to download the results as a document and attach to the encounter in OP. This may not be necessary, since the results should eventually flow into OP.
