Clinical staff may invite educators to take a CHADIS questionnaire through Office Practicum with a few additional steps. A questionnaire from teachers provides clinicians with valuable information to diagnose and manage health or developmental/behavioral concerns. Teachers will be able to securely log into CHADIS and complete questionnaires. The results will be available to the providers in Office Practicum.

Once teachers have been registered to CHADIS, they will complete questionnaires that were assigned via an order from OP. Teachers will not use the OP patient portal and will instead have a direct CHADIS account.

1. Add the teacher questionnaires via OP template order or manual order.

2. Next, the teacher is invited to register for CHADIS by staff, see Invite Teachers to CHADIS. Once the teacher has registered for CHADIS, they will be connected to the student and see the assigned questionnaires.
