It is the policy of the American Academy of Pediatrics that teenagers should provide information to their doctor independently of their parents, and there are many self-report questionnaires available in CHADIS for teen depression, substance use, anxiety, etc.
View the OP integration adolescent workflow options for an overview. One option is through providing child access to the primary caregiver account.
Self-report questionnaires can be assigned to the Primary Caregiver on CHADIS, and the child can complete the questionnaire through the primary caregiver’s OP portal. However, this is only recommended for questionnaires such as the SCARED, which allows parental assistance for younger ages (under 10yrs). Responses for teens are not confidential using this method.
Workflow:
Add the selected questionnaires to the OP templates.
Questionnaires will be assigned to the primary caregiver OP portal account automatically, when the parent receives the reminder for the visit.
On CHADIS, all self-report questionnaires have a warning message when assigned to a non-self respondent that it should be taken by the patient. This may prompt the primary caregiver to allow the patient to take their own questionnaire.
The child will take the questionnaire on the primary caregiver’s OP portal account. These results will appear in the patient chart in OP.
Considerations:
The primary caregiver can view these responses because it is in their own portal account - the answers are NOT confidential
Results show up in OP under primary caregiver’s name with nothing to indicate whether the child completed them
The warning message does not guarantee that the primary caregiver will allow the patient to take it, and they could end up mistakenly taking it for themselves
