If the respondent does not complete their questionnaires before the appointment, CHADIS questionnaires can be completed in the office on a phone or tablet by logging into their PCC patient portal OR by using the CHADIS ‘Waiting Room Workflow’. The CHADIS Waiting Room Workflow allows the respondent to access CHADIS questionnaires quickly without typing in their username or password.
