If you would like to assign questionnaires to adolescents, make sure to edit your auto-assignments in CHADIS to include self-report questionnaires for the desired teen ages. When the patient is approaching this age, primary caregivers or staff can invite the teen to set up their own CHADIS account and take questionnaires which have been assigned to Self. View Invite Teens to CHADIS (Respondent Instructions) for instructions to provide patients/families. Below are the instructions for staff to invite a teacher. 

1. Log into CHADIS and click on ‘All Patients.’

2. Search for the patient using name or DOB.

3. Click on the patient to open up the CHADIS patient file. Scroll down to the respondents section and click on the plus sign to register a teen respondent.

4. Choose the relationship 'Self/Patient' and CHADIS will fill in the patient’s name. Enter the teen’s phone number or email address, and click ‘Next’ to send them a registration message inviting them to use CHADIS.

5. A CHADIS registration message has now been sent to the teen. Next, click ‘Auto-Assign Questionnaires’ to auto-assign questionnaires to the teen.

6. Choose the Provider and the Visit Type and click ‘Save’ to assign questionnaires. Leave ‘Send assignment notification’ checked to send them a notification to complete questionnaires in CHADIS.

7. Self-report questionnaires will be assigned based on the patient’s age and the respondent relationship (based on the questionnaires you have set up in your auto-assignments) and appear in the ‘Questionnaires’ section of the CHADIS patient file.

8. The teen will receive a registration message for CHADIS and complete the assigned questionnaires once they have logged in.
