Add Patient (CHADIS Direct Clients)

The CHADIS Direct Staff-Driven Workflow requires staff to add patients to CHADIS directly.


  1. Log into CHADIS and click on the plus sign next to ‘Patients’ in the left column.

You can also click on ‘Patients,’ then ‘Add a Patient’.

2. Fill in the patient information including Name, DOB, and gender and click ‘Save.’

3. The patient has now been added to CHADIS and you can complete various actions from the CHADIS patient file such as as adding respondents or assigning questionnaires.

4. If you would like to edit any patient information, click on the 3 dots in the upper right corner of the patient section, and choose ‘Edit.’
