All respondents who have portal accounts (primary caregivers, adolescents whenever possible) should always be added to CHADIS through the integration. They will be automatically synced to CHADIS and this process is outlined in How Patients and Respondents Are Registered in CHADIS. However, there may be instances where a respondent needs to be added manually to CHADIS. See Teacher Workflow and Adolescent Workflow for specific workflows regarding teacher and adolescent respondents.
1. Log into CHADIS and click on ‘All Patients.’
2. Search for the patient using name, DOB, or PCC ID#.
3. Click on the patient to open up the CHADIS patient file. Scroll down to the respondents section and click on the plus sign to manually add a respondent.
4. Enter the respondent’s name, relationship, email address or phone number, and click ‘Next’ to send them a registration message inviting them to use CHADIS.
5. A CHADIS registration message has now been sent to the respondent. You can assign questionnaires or use the ‘Waiting Room’ options from this page if needed.
6. The respondent will receive a registration message for CHADIS that includes a link to complete their registration and take questionnaires.
