Manually 'Auto-Assign' Questionnaires (PCC Integrated Clients)

Staff can also manually assign questionnaires using the CHADIS auto-assignments that are pre-configured. This is useful if you would like to manually assign all questionnaires in a visit type to a particular respondent. For example, a patient came in for a well check visit and completed their assigned questionnaires, but the provider would like to assign all questionnaires that are typically assigned for an ADHD appointment.


  1. Log into CHADIS and click on ‘All Patients.’

2. Search for the patient using name, DOB, or PCC ID#.

3. Click on the patient to open up the CHADIS patient file. Click ‘Assign Questionnaires’ at the top of the patient file and choose ‘Automatic.’

4. Choose the respondent, the provider, and the visit type that contains the auto-assignments you would like to assign. Leave ‘Send assignment notification’ checked to send the respondent an email and/or text notifying the respondent that they have questionnaires to complete.

5. Once you click ‘Save,’ CHADIS will assign the questionnaires in that visit type that fit this patient’s age and respondent relationship and they will appear in the ‘Questionnaires’ section.

6. The results will appear in the messaging queue, shown inManually Assigned Questionnaire Results.
