If questionnaires were not assigned to the patient when you expected them to, it could be due to some of the reasons below.
The PCC visit reason is not mapped to a CHADIS visit type.
The patient did not meet the age criteria based on the CHADIS auto assignments. (Age is calculated at the time of assignment, not the appointment - 7 days ahead)
The prematurity field was populated with a value; therefore, CHADIS will age adjust for developmental vs. chronological age, up to age 2, per AAP guidelines. See Prematurity Adjustments.
The respondent’s relationship to the patient is not entered correctly for the PCC portal user.
A respondent for the patient (that is a portal user) was manually removed on CHADIS. This will cause a syncing error between PCC and CHADIS and no questionnaires will be assigned to any respondent for the patient.
The appointment was canceled or rescheduled and you are having trouble finding the results. See the PCC Learn Article for more information.
The questionnaires are expired. Questionnaires in CHADIS expire if left incomplete by the respondent after 30 days. It is important to consider this when rescheduling an appointment on PCC - if the questionnaires have already been assigned but not completed, they will remain assigned to the respondent and will not re-assign closer to the new visit date. If the new appointment date is more than a few weeks out, it is best to cancel the appointment and create a new one so that new questionnaires will be assigned closer to the new visit date. You can view ‘Questionnaire History’ in the CHADIS patient file to see expired questionnaires. *Ask your account manager if you would like to turn off questionnaire expiration in CHADIS.
