If you would like to assign questionnaires to adolescents, make sure to edit your auto-assignments in CHADIS to include self-report questionnaires for the desired teen ages.

Example:

When the patient is approaching this age, you want to invite them to set up their own CHADIS account and take questionnaires which have been assigned to Self.
