The CHADIS Direct Respondent-Driven Workflow requires patients and respondents to register themselves using your office invitation code.
1. Set your office invitation code during implementation with your CHADIS Account Manager as your 10 digit office phone number. You can create a pre-filled link with the code already filled in, or you can provide the code to respondents to enter on their own.
2. Send out notifications to your patients and families to register for CHADIS. See Sample Welcome Letter you can use as a handout, Sample Verbiage for Clinic Website to use the pre-filled link, and How to Register for CHADIS to attach an instructional handout for respondents.
3. The respondent clicks on your pre-filled link or visits www.chadis.com to enter their information and register themselves and their patient.
*If the respondent used your pre-filled link, skip to step 4 - the link will take them right to the registration page with the invitation code already filled in.
4. The respondent enters your office invitation code (if not already filled in) and respondent information and clicks ‘Register’ to set up their CHADIS account.
5. The respondent receives their login information and clicks ‘Continue.’
6. The respondent enters the patient information and clicks ‘Next’.
7. The respondent has created the patient and their account and can now take questionnaires or access any other features in their CHADIS portal.
8. If the respondent needs to add an additional patient, they can click ‘Return to Patient List.’
9. Next, the respondent clicks the green ‘plus’ icon in the top right corner of the patient list.
10. The respondent repeats steps 6-7 to register the additional patient and complete questionnaires.
