If you choose the respondent-driven workflow for your office, you will need to provide patients and families with reminders to log in to CHADIS and complete questionnaires. You can turn on the CHADIS Well Check Reminders or you can add verbiage to your general appointment reminders to ask them to log in. 

Below is sample verbiage to add to your general office appointment reminders to remind patients/families to log in to CHADIS and complete their questionnaires. These instructions are to initiate the respondent-driven auto-assignment process and you can also attach the How to Take Questionnaires handout (download using the link to the right).

Sample Verbiage for Appointment Reminders:

Make sure to visit www.chadis.com to complete health questionnaires before your upcoming visit. Please click ‘Login’ in the top right corner of the page to log in and select your patient. Click ‘Take Questionnaires,’ choose your provider, and choose the type of visit you are coming in for. Thank you for helping us provide the best care possible by answering important health questions for your appointment.
