The ideal ‘Gold Standard’ workflow for adolescents to complete questionnaires is through their own PCC portal account. This method ensures that the teen’s responses are kept confidential and the integration works as intended.
Workflow:
1. Add self-report questionnaires to your auto-assignments in CHADIS. Make sure to choose ‘Self/Patient’ as the relationship and set the appropriate teen ages.
2. Create a PCC portal account using the teen’s email address or phone number, and choose the relationship ‘Self.’
3. Teen receives notification via email or text to complete questionnaires through their portal 7 days before the visit, following the process outlined inAutomatic Questionnaire Assignment Process.
4. Teen may also complete questionnaires in the office if needed - through their portal or the CHADIS waiting room workflow, outlined in Waiting Room Workflow.
5. Results flow directly into PCC encounter under the appropriate screening order, shown in Auto-Assigned Questionnaire Results.
Considerations:
Consider setting a policy in the office that all teens must have their own portal account at a specific age
Have the front desk collect teen email addresses and phone numbers
Use suggested verbiage from CHADIS to inform parents of the importance of allowing teens privacy for their questionnaires
The portal account needs to be created for questionnaires to be automatically assigned, but they can access CHADIS through the waiting room workflow even if they do not accept their portal invite or log in
