This message is a request from a child's doctor for your observations to assist in that child's medical care. It is being sent to you by the child's parent or at their request. The parent or guardian has given permission for you to provide this information. You will be able to see the child's name and parental permission after you enter the secure website shown below. Anything you enter is confidential and seen only by the clinician and healthcare staff.

Information from family and teachers is very important. For example, research shows that regular feedback from parents and teachers allows for adjustment of ADHD medications, resulting in twice the reduction in symptoms.

* First time users - click the link below, locate the New User section, register and keep the logon and password for future use, then complete any questionnaires shown.
* Returning users- click the link below, locate the Returning Users section, login with your user name and password, then complete any questionnaires shown.

Please follow the instructions here depending on your role with this child:

*Parent*: Please complete the questionnaires unless they have 'Teacher' or 'Teen' in the title. /Also/, please forward this message to each of your child's teachers from whom you want input or to the guidance counselor or lead teacher for distribution. Forwarding this message gives your consent for those people to contribute information. You may also forward this link to other family members you wish to contribute to the assessment including to a child of 13 or over.

*Teacher*: Please complete this 3-minute online questionnaire to share your observations about your student. Please choose the appropriate 'For Teachers' option when asked.

* The first time: complete 1 'Teacher Classroom Behavior Rating'
* Subsequent times: complete 1 'Vanderbilt Follow-up, Teacher'

*Teenager*: Please complete any questionnaires that say 'For Teen'. Remember that no one besides the doctor and staff can see your answers.
