Check if questionnaire was completed

How do you know if the questionnaire has been completed?
1. From the Home screen within the CHADIS system, enter the patient’s name in the search window and hit the "Enter" key on your keyboard.
2. Select the patient you are looking for by clicking on the magnifying glass to the left of the patient’s name
3. The details window for the patient you have selected will then appear.
4. Scroll down to "Questionnaires".
5. You will see a grayed out "X" next to each completed question along with the word “Submitted” under the status sections. If they have not yet been submitted they will have a Red "X" and be in "New" in the status section.
