It is the policy of the American Academy of Pediatrics that teenagers should provide information to their doctor independently of their parents, and there are many self-report questionnaires available in CHADIS for teen depression, substance use, anxiety, etc.
Below are the different options for patients to complete self-report questionnaires through the CHADIS/OP integration.
Teen accesses questionnaires through their own OP portal account (‘Gold Standard’)
Teen accesses questionnaires through a CHADIS Direct account
Child accesses questionnaires through a primary caregiver’s OP portal account (only recommended for younger ages)
