Q: How do I access questionnaires at the time of service or conduct follow-up questionnaires, e.g. M-CHAT R Follow Up Questionnaire?
A: Providers and staff will be able to access CHADIS directly via sign in at chadis.com. Once in CHADIS, you can manually assign questionnaires or complete follow up questionnaires. Results from follow-up questionnaires will be returned to PCC within moments of the visit.
Q: Are all CHADIS questionnaires available via the integration?
A: All questionnaires in CHADIS are supported by the integration. The PCC integration supports all questionnaire results returned to the patient's chart.
Q: How are questionnaires assigned to teachers and how do they complete them?
A: Staff or parents can invite a teacher to complete a questionnaire directly through CHADIS under the specific patient. The teacher will be notified via email/text to create a CHADIS account and complete the questionnaires assigned to them. Results will be returned to PCC via the messaging queue once the questionnaire is completed. See Teacher Workflow for more information.
Q: What happens when a new version of a questionnaire is available?
A: CHADIS will notify you of any new versions and work with you to update your automatic assignments. You must make sure to update your screening orders in PCC to reflect the changes made to your auto assignments in CHADIS.
Q: How often does CHADIS check the schedule?
A: CHADIS does not check the schedule. PCC checks the schedule and sends appointment details to CHADIS. PCC will sync appointments to CHADIS every few minutes to assign CHADIS questionnaires.
Q: What if parents/patients don’t complete their questionnaires before a visit?
A: Questionnaires can be completed in the waiting room via a computer, tablet, or mobile device. Parents can log into the PCC portal from any device and click the CHADIS button to see and complete the questionnaires. Respondents can also complete questionnaires directly through CHADIS using the waiting room workflow. See Waiting Room Workflow for more information.
Q: How quickly will results be returned if the questionnaire is completed in the waiting room?
A: CHADIS processes questionnaires very quickly and will return the results to PCC within 5 minutes of the questionnaire being submitted so the provider can review the results during the appointment.
Q: Where are the results of the questionnaires returned or displayed in PCC for automatic assignments?
A: Questionnaire results for automatic assignments are displayed in the screening order in the visit encounter for the patient as soon as the respondent completes the questionnaire. Scores/results will appear directly under the appropriate screening order. The blue ‘Details’ link next to the screening order will display the questions/responses to the questionnaire. Q: Where are the results of the questionnaires returned or displayed in PCC for manual assignments and teachers?
A: For manual assignments and teacher questionnaires, these results will appear in the PCC messaging queue under an ‘unassigned user.’ From here, you can open the result and attach it to the appropriate patient encounter. See Manually Assigned Questionnaire Results for more information.
Q: What if a patient in PCC does not have an appointment scheduled, but I want them to take a questionnaire?
A: Patients and their respective portal users in PCC will automatically sync to CHADIS if a.) they are an active patient and b.) there is a portal user assigned to the patient (they do not need to have a scheduled appointment). You can manually order CHADIS questionnaires for a patient by logging into CHADIS and adding the questionnaire to the patient. Q: What if a patient cancels an appointment that has CHADIS questionnaires?
A: If a patient cancels an appointment and the questionnaires have already been completed the results will remain in the canceled appointment in the PCC patient chart. These results can be moved to a new appointment if needed (see PCC Learn Article). If the questionnaires have not already been completed, they will be canceled, and a new set of questionnaires will be sent to the patient when a new appointment is scheduled.
Q: What if a patient reschedules an appointment that has CHADIS questionnaires?
A: If a patient reschedules the appointment (and you use the ‘reschedule’ option on PCC), the orders and results will remain with that appointment. New questionnaires will not be assigned.
